This Sweepstakes is open only to legal residents of New York State who are at least 18 years of age at the time of entry. Employees of Cobble Hill Health Center (“Sponsor”), its affiliates, subsidiaries, advertising and promotion agencies, and immediate family members are not eligible. Void where prohibited by law.

The Sweepstakes begins on March 1 and ends on May 1 (“Sweepstakes Period”). All online entries must be received by the end of the Sweepstakes Period.

Email Sign-up: Visit cobblehilllifecare.org and follow the instructions to sign up for our email newsletter. By signing up, you will receive one (1) entry into the Sweepstakes. You may opt out of marketing emails at any time without affecting your entry.

Limit: One (1) entry per person. Multiple entries from the same person will be disqualified.

Prize: One (1) winner will receive an Apple Watch Series 11 [GPS + Cellular 42mm] (Approximate Retail Value: $499.00). The prize is non-transferable and no cash redemption or substitution is permitted, except at the Sponsor’s sole discretion. Winner is responsible for all federal, state, and local taxes.

One (1) potential winner will be selected in a random drawing on or about May 11 from all eligible entries received. The potential winner will be notified via the email address provided at entry within 30 days of the drawing. If a potential winner cannot be reached or does not respond within 30 days, an alternate winner may be selected.

The odds of winning depend on the number of eligible entries received.

By participating, you agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding. Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud or technical failures corrupt the administration of the giveaway.

By entering, you release and hold harmless the Sponsor and its affiliates from any liability, illness, injury, or damage that may occur from participation in the Sweepstakes or use of the prize.

To obtain a copy of the winner's name, mail a self-addressed, stamped envelope to: 380 Henry St, Brooklyn, NY 11201 within 30 days of the end of the Sweepstakes.
